The majority of the cost for services delivered by Crossreach is normally paid for by the Government of the Province of BC. These funds are administered by the Vancouver Coastal Health. However, clients are required to pay the following daily costs:
HandyDART (daily) if required
Meal and program (daily) $ 10.00
Membership Fee (annually) $10.00
ELGIBILTY:
The guidelines for eligibility include the following:
· Resident of the province of BC for six months.
· Be assessed by the local health unit.
Crossreach was founded in 1973 to provide support to seniors, their families and their caregivers who were finding it difficult to maintain an independent lifestyle due to issues arising from the aging process. Over the past 45+ years, our organization has expanded both the scope of the services provided and the number of seniors served. At the present time our main focus is to operate an Adult Day Center as part of the health care system funded by the Province of BC through the Vancouver Coastal Health Authority. Currently we provide services to approximately 65 seniors.
To be eligible to participate in Crossreach as a client:
- be 65 plus
-Live in the West Point Grey area of Vancouver (including Kitsilano, Kerrisdale, Dunbar and Point Grey)
- be referred to us by a case manager or a referring agency
-meet with a representative of the Vancouver Coastal Health Authority, who will assess your needs and determine if our services will be of benefit, and
- be issued a long-term care number by the Health Authority
